Dear Participants:
The following forms are set by the programs you have requested to be enrolled in. All forms in each program must be printed and completed in full, all areas signed and the Original forms must be mailed to Kids Sea Camp.inc (P.O. Box 291030, Columbia, SC 29229). Final documents will not be sent until all forms are provided. Please make sure to read all forms carefully, if you have any questions or concerns please contact us. (803-419-2556).

Kids Sea Camp, Inc.
FAMILY APPLICATION
(For all Adults & students ages 4-15)

Dear Parents,
Forms 1, 2, & 3 are required to be sent in with your deposit to confirm space.
1. Kids Sea Camp Application - Must be filled out and faxed, mailed or emailed to Kids Sea Camp to confirm space.
2. 2010 Kids Sea Camp Terms & Conditions - One per family, must be carefully read, filled out and returned to Kids Sea Camp to confirm space.

3 . Kids Sea Camp Credit Card Form - Kids Sea Camp can not accept any credit card payments without Authorization form completed.
4. Kids Sea Camp Equipment Request Form - Kids dive gear rental is included in your package, however we do request sizing. If adults require gear rental complete this form so that we may reserve your gear rental request. Gear rental for adults is not included in your package.

FORMS

These are standard PADI forms for all scuba diving activities. It is important that all information be provided. If you or your child has any medical condition or is taking any medications proscribed by a doctor, you must provide a doctors note of approval to participate in requested activities. Kids Sea Camp and its operators make every effort to accommodate every child. Kids Sea Camp has proudly been able to provide great family vacations for deaf children, autistic children, handicap children, children with Asthma and Arthritis, as well many other conditions. When all information is provided with enough notice, we make every effort to meet special needs. This is not possible in all locations, but communication is the key.

Kids Sea Camp, PADI Dive operators, all employees and instructors and dive professionals with or in association with PADI and Kids Sea Camp reserves the right to deny any request based on the PADI operators approval, willingness and ability to provide a safe activity for you or your child at selected destination.
